Meeting Registration Form
To register for an SWPC meeting please complete the form below and you will be taken to a page where, if relevant, you can pay online via PayPal. If you are unable to do this you can print and complete this registration form and send it with your cheque to the SWPC Treasurer, Dr Laraine Dibble, Child Health, Torbay Hospital, Torquay, TQ2 7AA.
